The SEX-SHOCK Score: An Opportunity for Early Intervention in Cardiogenic Shock?

Commented by the ESC WG on Atherosclerosis and Vascular Biology

Clinical
Vascular Diseases

Cardiogenic shock (CS) remains a leading cause of in-hospital mortality following acute coronary syndromes (ACS), with existing risk prediction tools demonstrating suboptimal performance in females. This multinational, large-scale study introduces the SEX-SHOCK score, developed using machine learning and regression-based approaches, now relying on novel predictors of CS in females and males: ST-segment elevation, creatinine, C-reactive protein, and left ventricular ejection fraction. The newly developed SEX-SHOCK score significantly improves risk prediction in both sexes and outperforms previous risk prediction models across healthcare systems, nations, and clinical settings.

The online calculator is now available, ready for clinical use, allowing quick and personalized risk assessment of patients with ACS. The SEX-SHOCK score holds immense promise to enhance future clinical decision-making and personalized interventions in patients at risk of CS.
