Artificial sweeteners such as xylitol are widely used to replace dietary sugar in patients with cardiometabolic disease. However, their long-term effects on vascular complications are unknown. Using untargeted metabolomics and stable isotope dilution liquid chromatography tandem mass spectrometry, fasting levels of xylitol were associated with cardiovascular events. Both in vitro, in a murine arterial injury model in vivo, and in healthy volunteers xylitol enhanced platelet responsiveness and thrombosis!
